Our plans provide coverage for a wide range of health care and medical services including a very impressive preventive
health program. Doctor office visits cost $20 or $25/40 depending on the co-payment plan the group chooses. You can select from:

Plan 25/40 Enhanced. Pay a $25 co-payment for office visits and $40 co-payment for specialist visits. There is no co-payment for hospitalization. |

Plan 25/40 Low Option. Pay a $25 co-payment for office visits and $40 co-payment for specialist visits. There is $500 co-payment for hospitalization. |

In addition, there are a variety of riders available to you and your employees which offer a number of prescription drug benefits, including mail order, vision benefits and enhanced benefits for mental health/substance abuse and skilled nursing facilities.
