Our plans provide coverage for a wide range of health care and medical services including a very impressive preventive health program. Doctor office visits cost $10, $15, or $20 depending on the co-payment plan the group chooses. You can select from:

| Plan 10E | Plan 10 Enhanced. Pay a $10 co-payment for office visits. There is no co-payment for hospitalization. |
| Plan 15E | Plan 15 Enhanced. Pay a $15 co-payment for office visits. There is no co-payment for hospitalization. |
| Plan 20E | Plan 20 Enhanced. Pay a $20 co-payment for office visits. There is no co-payment for hospitalization. |
In addition, there are a variety of riders available to you and your employees which offer a number of prescription drug benefits, including mail order, vision benefits, student coverage and enhanced benefits for mental health/substance abuse and skilled nursing facilities.
